Term
Jaundice
Definition
bilirubin is excreted through bowel
Term
Jaundice reasons
Definition
Hemolytic-excessive breakdown of RBCs (due to sickle cell or blood transfusion reaction-incompatibility)
Hepatocellular-liver cannot breakdown RBCs; conjugated/unconjugated; most common
Obstructive-problem in excretion phase, usually in ducts, clay colored stool (ex. tumor, cancer)
Term
Conjugated vs. unconjugated bilirubin
Definition
total bilirubin=conjugated (excreted) + unconjugated (non-excreted)
high conjugated=liver is working properly
high unconjugated=liver not working properly
Term
Hepatitis
Definition
inflammation that alters function of liver
Term
Types of Hepatitis
Definition
Hep A - fecal to oral route
Hep B - blood borne
Hep C - contaminated needles; unprotected sex; HIV
Hep D - secondary infection AFTER Hep B
Term
Acute vs. chronic hepatitis
Definition
IGM - acute
IGG - chronic past infection
Term
Acute Hepatitis
Definition
IGM; malaise, RUQ abdominal discomfort; headache; low fever; dark amber urine; clay colored stool
Term
Convalescent Hepatitis
Definition
type of remission;general malaise
Term
Hepatitis Complications, DX, TX
Definition
Complications: progressive liver failure, cirrohsis, cancer
DX: test antibodies for each type
TX: goal is to eradicate Hep C virus; drug therapy in hopes to sero convert; Interferon-blocks entry into cells; Baraclude-chronic Hep B with active viral replication; watch CD4 counts (over 200=continue TX; less 200=hold off)
Term
Hepatitis Nutrition, care, prevention
Definition
Nutrition: general healthy diet without alcohol; avoid hepatotoxic drugs (Tylenol)
Care: symptom relief/management; prevention
Prevention: teach routes of admission (safe sex, handwashing); Hep A-vaccine, Hep B-vaccine with titres
Term
Primary Biliary Cirrohsis
Definition
chronic inflammation of liver; T cells attack the bile duct cells causing blockage; associated with RA, Lupus; 95% women 30-60
s/s: itching, fatigue, high alk/phos, high ANA, Later jaundice
TX: Actigel, Questran, ERCP-see if common bile duct is open and patent, if blocked, can shunt
Term
Non-alcoholic Fatty Liver Disease
Definition
major complication of obesity, related to poor diet, TB, intestinal bypass, corticosteroids
s/s: asymptomatic
DX: can see fatty infiltrates on CT scan or ultrasound
TX: decrease weight factors, stable slow weight loss (or can go into liver failure), improve and stop progression not reverse completely
