- Influences every body system by increasing metabolic rates increasing heart rate, RR, temp, CO,O2 consumption and metabolism of nutrients. How this occurs is not well understood.

- levothroxine (Synthroid) is drug of choice for hypothyroidism bc relatively cheaper, once a day dosage and more uniform potency
- used in conjunction with antithyroid treatment for thyrotoxicosis (thyroid destroyed, and hormones replaced)
- used as a diagnostic measure to differentiate hypothyroidism from euthyroidism
- NURSING ALERT – variation exists bt brands. Do nto change brands or type of hormone without approval; may need to change dose if changing brand/type

- once a day, preferably before breakfast
- empty stomach enhances absorption of oral form
- diabetics at risk for hyperglycemia (increases diabetic sxs); if hyperglycemic, notify PCP
- Pts with CV disease – monitor for chest pain or worsening CV disease and report to PCP immediately bc may need dose reduction
- Elderly at increased risk for adverse CV reactions, so their initial dose is smaller and increased as needed for 8 weeks or so
- levothyroxine and liothyronine can be given IV

- Takes time to get dose right (for pt to become euthyroid), so pt may either see no improvement, or have adverse reactions from hyperthyroidism
- Monitor reaction closely to help with dose adjustment
- NURSING ALERT – signs of hyperthyroid reported before next dose bc may need reduction
- “Anxiety rt symptoms, adverse reactions, treatment regimen, etc”
- concerns over symptoms of disorder, and concerns about relief of sxs
- reassure that relief not immediate, but will get better in several weeks

- Therapy is evaluated at intervals; about 2 weeks initially, then q 6-12 months

- For hyperthyroidism
- Inhibit manufacture of thyroid hormone
- does not affect existing hormone already made, either circulating or stored at thyroid.
- This is why it takes weeks for therapeutic effect (takes a while to use up stores)
- Radioactive iodine used bc the thyroid has affinity for iodine; the radioactive accumulates at thyroid and destroys thyroid tissue without damaging other body tissues
- Not all pts respond adequately to anti-thyroid drugs and may need surgical removal
