(PFJS)
MOI/ CAUSE /DX TEST /DIFFDX / TX

|
Definition
- IND:ANTERIOR KNEE PAIN
- MOI:Patella displaced/dislocated laterally due to twisting or jumping, audible 'pop'
- CAUSE: Hxtrauma or lig laxity/other biomechanical predispositionb (q angle, valgum, pronation)
- DX TEST: Tender Medial border Patella, Positive Lateral Apprehension test (watch facial expression & pain on quads Ctrn) & Clarke's (pain on Ctrn). Pain 1 leg squat.
- DIFF DX: ACL rupture, Ostochondral # (xray r/o)
- STRETCH: Quads, lateral structures if tight
- STRENGTHEN: VMO, CORE /pelvic stability
- TAPE: McConnell- Medial glide w hyperfix, see wrinkle medially. 1-2days.?Inferior tilt/Lat rotation

|
Definition
- IND: ANTERIOR KNEE PAIN exacerbated with loading activities, common at start and end of activity (cf PFJS inc during)
- MOI:jumping, repetitive eccentric load, fall on knee
- DX TEST: INFERIOR PATELLA tender, Single/Dbl leg squat, weaks quads, U/S & MRI can view
- DIFF DX:Infrapatella pad fad impingemnt(c.f as this occurs with forced extension, no eccentric load eg kick in swimming)/bursitis, PFPS
- STRETCH:
- STRENGTHEN: Quads.
- TAPING: Band around inferior pole

ITBFS (ILIOTIBIALBANDFRICTION SYNDROME)
MOI/CAUSES/DIFFDX/DX TEST/TX
[image] |
|
Definition
MOI: Running esp downhills. friction of ITB over lat epicondyle femur due to less flexion during running. Flexion of knee allows for less friction.
CAUSES: pronation,Tight/wideITB,lat pelvic tilt
DX TESTs: OBER's - positive ind tight ITB, MRI & U/S, tenderness over lat epicondyle 2-3 cm above lat JT line, crepitus may be felt, pain on repeated flex/ext
DIFF DX: LCL strain, FIB #, LAT MENISCIS, BURSA( ITB adventitious bursa may occur simultaneously)
STRETCH: ITB ?adductors
STRENGTHEN: ABDUCTORS (Med/Min)
ORTHOSES: Low dye tape then Antipronatory |

(MEDIAL TIBIAL PERIOSTITIS)
MOI/CAUSES/DX/DIFFDX/TX |
|
Definition
MOI: Running causing inc stress/traction on tibia
CAUSES: Pes Planus, Tibial varum during running, FDL. TB & SOLEUS Implicated, surface training error, <BMD
DX:Tenderness at Posteromedial Tibial Border. Unlocalised Diffuse pain.Worse in morning & after exercise. MRI: periosteal thickening/ oedema, Bone Scan:diffuse uptake. Improves w warmup/stretch. TEST: HOP/RUN/CALF RAISE
DIFFDX: Compt Syndrome, Tib Post Dysnfn, Stress #, Popliteal Artery Entrapment, Mm/Tendon injury
TX: RICE, non wb exercise, Biomechanical Exam -Tape only if pronation, Transverse friction massage, Digitall Ischaemic (p). PNF Strengthening & flexibility WPL |
