She clutches her chest to show how pained she is, but may wink at the audience to show them that she is only pretending.

What do you do once you have explained why the line should be performed this way?

You then explain what effect this will have on the audience and maybe the other characters on stage.
The exaggeration and wink of the eye will let the audience understand that the nurse is being mischievous and having a joke with Juliet. It may well make them laugh and join in with her joke. They may find it funny that Juliet cannot see this. Juliet will not notice the nurses humour as she is still too caught up with trying to find out her answer as quickly as possible.
