Term
rebound congestion patient teaching
Definition
Don’t take for more than 3 days! Call if you have problems. Why is this important? Sleep issues, nasal issues, etc.
Drug is absorbed very quickly because of its direct placement on mucous membranes, inflammatory process happens again after drug wears off
Can cause addiction
Term
decongestant side effects
Definition
(Sudafed, neosynephrine): nervousness, insomnia, palpitations, tremor. Most common for intranasal steroids are localized and include mucosal irritation and dryness. Excessive doses can cause cardiovascular effects (hypertension, palpitations) and CNS effects (headache, nervousness, and dizziness) systemic effects are the result of alpha-adrenergic stimulation of the heart, blood vessels, and CNS
Term
Expectorant side effects
Definition
(mucinex, guiafinosen, Robitussin, Mucomyst) – don’t swallow sputum! Adverse effects are minimal. Guaifenesin may cause nausea, vomiting and gastric irritation.
Term
Antihistamine side effects
Definition
(Allegra, Chlor-Trimeton, Benadryl, diphenhydramine, loratadine, Zyrtec – old vs. newer) Drowsiness is usually the chief complaint of people, but the sedative effects vary from class to class (less common). Anticholinergic (drying) effects of antihistamines can cause dry mouth, changes in vision, difficulty urinating, and constipation.
Term
Side effects of bronchodilators
Definition
increased HR, jittery, palpitations, restlessness, insomnia, increased BP, tremors, anxiety
Beta adrenergic can be mixed/ alpha beta, nonselective beta, or beta 2. there's also anticholinergic and xanthene derivaties
Term
Corticosteroid side effects
Definition
paryngeal irritation, coughing, dry mouth, and oral fungal infections. instruct patients to rinse their mouths after use of an inhaled corticosteroid.
Term
multiple inhaler process
Definition
1. use bronchodilator first to open airway
2. wait 5-10 minutes
3. use steroid inhaler
4. Rinse out mouth
Term
Rescue drug
Definition
Albuterol- selective beta 2 drug, which relaxes smooth muscle in airway and results in bronchial dilation and increased airflow. given during acute asthma attacks
Term
maintentance drug
Definition
LTRA's rpevent leukotriene's from attaching to receptors on cells in the lungs and in circulation, inflammation in the lungs in blocks, and asthma symptoms are relieved. given for long term treatmetn and prevention of asthma
Term
xanthene derivaties mechanism of action
Definition
cAMP plays integral role in maintaining airway
inhibit phosophodiasterase so that there's more cAMP in the body. this will cause smooth muscle relaxation and inhibit IgE induced release of chemical mediators that drive allergic reactions, which increases bronchodilation
Term
side effects of xanthene derivatives
Definition
NV, anorexia, gastroesophogeal reflex during sleep, cardiac adverse effects, increased urination and hyperglycemia
Term
amniophylline toxicity
Definition
metabolized to theophylline and then caffiene
stimulatory effect
use repeaded administration of doses of activated charcoal
Term
Side effects of oral antidiabetic agents
Definition
hypoglycemia
GI irritation
decreased Urine output
jaundice
Term
aspart insulin
Definition
fast acting: facilitates uptake of excess glucose at hepatic insulin receptor sites for storage in the liver as glycogen. onset 15 minutes, peak 1-2 hours, duration 3-5 hours.
Term
regular insulin
Definition
short acting, works by helping glucose get into cells
onset 30-60 minutes, peak 2.5 hours, duration 6-10 hr
Term
NPH/ insulin isophane suspension
Definition
intermediate acting: onset 1-2 hours, peak 4-8 hours, duration 10-18 hours
Term
glargine
Definition
long acting: provides a constant level of insulin by forming microprecipitates that are slowly absorbed over the next 24 hours. onset 3-4 hours, no peak, duration 24 hours
Term
highest risk for hypoglycemic reaction
Definition
after lunch before supper
Term
priority nurse actions for hypoglycemic reaction
Definition
oral forms (or buccal tablets or semisolid gel forms) of concentrated glucose if they're conscious
If they have an IV, 50% dextrose in water
If they're unconscious and don't have an IV, SQ injection of glucagon- lay them on their side first.
Term
nursing assesments for before endocrine agents
Definition
nursing history, VS, lab values
contraindications, weight
cautious with cardiac disease, hypertension, pregnancy
Term
corticosteroid use and principles
Definition
decrease inflammation for respiratory, chemo, transplants
wean and taper dose, decreasing systemic and increasing inhaled so that there's no adrenal suppression
take in the AM with water
avoid sick people/crowds
monitor for signs of infection
Term
corticosteroid susceptibility to infection
Definition
supresses the immune response by preventing inflammation
Term
Cushing's syndrome
Definition
oversecretion of adrenocortical hormones (from tumor or excessive administration of steroids)
causes body fat to move to face, shoulders, trunk and abdomen, which leads to moon face and camel hump

can result from prolonged or frequent use of glucocorticoids
Term
hypothyroidism and meds
Definition
diminished production of thyroid hormone (from thyroid abnormality, pituitary failure to secrete TSH, or hypothalamus failure to secrete TRH). treated with thyroid hormone replacement using preparations that are desiccated or synthetic (Synthroid most common)
Term
hyperthyroidism
Definition
caused by excessive production of thyroid hormones, can be caused by diseases like Graves or cushings. treated with antithyroid drugs like tazapole. drugs impede formation of thyroid hormone by not allowing precursors of T3 and T4 to be made. Also radioactive iodine or surgery
Term
biophosphonates
Definition
for osteoporosis- inhibit osteoclast- mediated bone resorption
take on empty stomach first thing in AM
stay upright for at least 30 minutes to avoid esophageal reflux
fosamax and boniva
SE: HA, GI, Joint pain
Term
Estrogen therapy patient teaching
Definition
don't smoke
Adverse, DVT, PE, Stroke -know S/S
SE: breast tenderness, hot flashes, weight gain, fluid retension, hypertension, nausea, thyrombolytic events, breast and ovarian cancer, ab. pain, Chest Pain, HA, eye problems, severe leg pain

report: hypertension, edema, thromboembolism, migraines, depression and breakthrough bleeding, EBP, headaches with changes in vision and vomiting, ab pain
