General

Definition

Very large cost to the health care system.

#1 nosocomial infx (30% of total)

#1 community acquired infx

 

50% of women are >60yo

Urinary Tract Infection:

Characteristics

Definition

Primarily a female population concern

80% considered to be "uncomplicated" with E.coli as offending bacteria.

10% caused by G+ staph species (Staph a. rising quickly)

Season patterns - patterns specific to individual communities 

Elderly have high mortality rate assoc. w/UTIs

When males get UTI → r/t structural abnormality

23% of women have no symptoms

Bacturia

Definition

Asymptomatic presence of bacteria - does not mean infx.

 

20% of female population at any given time.

UTI:

Cystitis

Definition

1st stage of noticeable bladder infx - Typically mild to moderate - easily treated, often self resolving. (30% have renal tissue involvement with no s/s = caution!)

Onset 24-48 hours post inoculation

Dysuria, pyuria, frequency. Maybe low grade temp.

Pyelonephritis

Definition

Infx of the renal tissue.

Acute or chronic - if chronic → could it cause permanent damage?

Need for aggressive Tx - often IV antibx needed x48 hours followed by oral course 7-10 days

E.coli 80% of the time

UTI:

Un/Complicated

Definition

Un/complicated - applies to all locations of UTIs. "Complicated" indicates a co-morbidity. Diabetes or obstructions are largest concern!

"Complicated" = less predictable: E.coli most common. Staph a., Pseudomonas, Klebsiella, Candida.

"Complicated" = full Tx regime: Bactrim, Septra #1 drug. Cipro, Levaquin backups.

Definition

Controls symptoms w/i 3-5 days. 

Broad spectrum bacteriocidal. Inhibits DNA production.

Does NOT cross BBB.

 

Very high degree solubility.

Caution with tendon in pts. ↓ 18 yo

 

AAPC - Antibiotic Associated Pseudomembranous Colitis

Sulfonamides

Definition

Broad spectrum bacteriostatic.

↓ Folic acid production → ↓RNA/DNA

High degree of solubility = high concentration in urine.

AE: Hypersensitivity, blood dyscrasias, urine crystal formation. Drug interact: coumadin, oral hypoglycemic agents, cross sensitivity w/Lasix and thiazides

Trimethoprim

Definition

Broad spectrum bacteriocidal/bacteriostatic.

↓ folic acid production → ↓ RNA/DNA.

High degree of solubility.

 

AE: GI tract, N/V, itching, rash, blood dyscrasias, anemia, neutropenia, thrombocytopenia.
