Term
what is Eosinophilic Granuloma Complex (EGC) in cats?
its ______mediated
are they pruritic?
do they have primary or secondary lesions
Definition
-this is not a specific disease, but rather a rxn
pattern.
-OFTEN ALLERGY-MEDIATED (atopy, food, flea,
mosquito).
-Pruritus is variable!!
-Often have primary lesions
Term
What are four syndromes cats with EGC get?
Definition
1. Eosinophilic plaques
2. Linear granuloma (eosinophilic granuloma)
3. Indolent ulcer (rodent ulcer, eosinophilic ulcer-
feline eosinophilic lip ulcer)
4. Mosquito bite Hypersensisitivity
Term
what are six common features among all four syndromes of EGC?
Definition
1. MAY BE ALLERGY MEDIATED (atopy, food, flea,
mosquito)->work up for allergies
2. EOSINOPHILS OFTEN PREDOMINATE
3. May see all 4 lesions in an individual
4. DDX: bacterial/fungal infection or neoplasia
5. Dx: one of exclusion (may need to biopsy to rule
out above DDX)
6. Tx: avoidance of relevant allergens when
possible; hyposensitization therapy for atopy-
induced lesions; most respond to anti-
inflammatory doses of prednisone; occasionally
respond to Ab
Term
what is the Tx for any of the four syndromes of EGC?
Definition
1. avoidance of relevant allergens when
possible
2. hyposensitization therapy for atopy-induced
lesions
3. most respond to anti-inflammatory doses of
prednisone
4. occasionally respond to Antibiotics
Term
what is the DDX for all four syndromes of EGC?
Definition
bacterial infection, fungal infection, neoplasia
Term
what cell predominated in EGC reaction?
Definition
eosinophils
Term
what is Eosinophilic Plaque in EGC most often caused by?
Definition
Flea Allergy Dermatitis
Term
what is the distribution of eosinophilic plaques in EGC? are they pruritic? are they dry or moist plaques? are other infections usually present?
Definition
-often inguinal and ventral abdomen
-very pruritic
-Moist plaques
-2ndary infection often involved
Term
what is Tx for Eosinophilic Plaques in EGC?
Definition
tx as for FAD
Term
what is the etiology for Linear granuloma (eosinophilic granuloma) in EGC?
Definition
-not usually associated with allergy
-may be genetic predisposition
-(cats<1 y/o spontaneously regress)
Term
describe the distribution of lesions in linear granuloma of EGC. shape? pruritic? location on body?
distribution
Definition
-linear, usually NON-pruritic
-caudal thighs, face, oral cavity of young cats
-may cause lower lip and chin swelling
Term
what is the tx for linear granuloma of EGC is non-pruritic? what if it is pruritic?
Definition
-if not Pruritic then "scientific neglect"
-if pruritic, short-term systemic steroids
Term
What are causes of Indolent ulcer in EGC?
Definition
some cases are allergy mediated
Term
where are the lesions in Indolent Ulcer of EGC?
are they pruritic? what so the lesions look like?
Definition
-lesions are on upper lips and in oral cavity
-usually non pruritic and non-painful
-lesions are well-circumscribed, red-brown,
alopecic ulcers
Term
what do painful oral Indolent Ulcer of EGC lead to?
Definition
dysphagia and/or anorexia
Term
what is tx for indolent ulcers of EGC?
Definition
-identify/trest specific allergy
-steroids if cannot accomplish this
-some may become refractory to steroids
-surgical removal if painful oral lesions are
refractory to medical treatment
Term
what causes mosquito bite hypersensitivity in EGC?
Definition
probably a Type I and/or Type IV hypersensitivity rxn
Term
what kind of lesions are present in mosquito bite hypersensitivity EGC and what is their distribution?
Definition
-eruptive moist lesions
-on face, ears, and margins of pads
Term
how do you treat mosquito bite hypersensitivity in EGC?
Definition
1. avoidance (house indoors, mosquito netting,
repellants)
2. steroids until resolution
