Albuterol (Ventolin/Proventil) Acute, beta adrenergic
Definition
Class: Bronchodilator
Action: Prevention and treatment of reversible bronchospasm associated with asthma and other obstructive pulmonary diseases. Produces bronchodilation by relaxing bronchial smooth muscle through beta 2 receptor stimulation.
Side Effects: Bronchospasm, Allergic reactions, irregular heatbeat, dizzy, headache lightheadedness, insomnia, tremor or nervousness, sweating, nausea.
Contraindications: Cardiac tachyarrhythmias.
Nursing consideration: Take with full glass of water. Wait two minutes between neb doses. Use 5 minutes before corticosteriod if ordered. Use 15 minutes prior to exercise.

Epinephrine acute, Beta adrenergic
Definition
Class: Bronchodilator, vasopressor, stimulant, topical antihemorrhagic.
Action: Relaxes bronchial smooth muscle, causes cardiac stimulation, relieves allergic signs and symptoms, helps stop local bleeding, and decreases pain sensation.
Side Effects: cerebral hemorrhage, nervousness, stroke, tremors, HTN, shock, palpitations, tachycardia, ventricular fibrilation, headache, drowsiness.
Contraindication: those with angle closure glaucoma, shock (other than anaphylactic), organ/brain damage, cardiac dilation, arrhythmias, coronary insufficiency, or cerebral rteriosclerosis, and those in labor.
Nursing consideration: When given IV, monitor vitals, EEG, Avoid IM into buttocks, Gas gangrene may occur because epinephrine reduces oxygen, tension of tissues. Massage site of IM injection, If blood pressure rises sharply, rapid actiing vasodilators, sush as nitrites or alpha blockers can be given to counteract effects.

Definition
Class: Bronchodilator
Action: Improves breathing
Side Effects: Headache, ventricular arrhythmias, nasopharyngitis, bronchospasm, worsening of asthma, anaphylaxis.
Contraindication: If hypersensitive to drug.
Nursing considerations: Assess respiratory status prior and thereafter. Not for acute bronchospasm.

Theophylline (Slophyllin) acute if not using extended release, xanthine derivative
Definition
Class: Bronchodilator
Action: Improves breathing ability.
Side Effects: Dizziness,insomnia, restlessness, seizures, arrhythmias, marked hypotension, nausea, vomiting respiratory arrest.
Contraindications: with active peptic ulcer or seizure disorders. Hypersensitive to xanthine compounds. Don't use extended release for acute bronchospasm.
Nursing considerations: Monitor patient, VS, I and O, dose determined by monitoring response. If drug not available give hot cup of coffee or tea Black. If theophylline levels are high, ask about coffee intake.

Definition
Class: Bronchodilator
Action: Relaxing smooth muscle of the respiratory tract, increasing airflow, and vital capacity.
Side Effects: arrythmias, headache, hypotension, IRITABLITY, nausea, PALPITATIONS, restlessness, urine retention, and VOMITING.
Contraindications: Hypersensitivity to drug.
Nursing considerations: monitor for toxicity, Vital signs.

Class: Antiinflammatory, immunosuppressant.
Action: Relieves inflammation and reduces immunosuppression.
Side Effects: Euphoria, insomnia, seizures, heart failure, thromboembolism, peptic ulceration, pancreatitis.
Contraindications: with systemic fungal infection, and allergy to any part of this drug.

Azmacort Profilactic, Corticosteriod
Definition
Class: Anti inflammatory, immunosuppressant.
Action: Improves breathing ability.
Side Effects: Headache, sneezing, oral candidiasis, hypothalamic-pituitary-adrenal function suppression.
Contraindications: With status asthmaticus, and allergy to any part of this drug.
Nursing considerations: watch mucus membranes for fungal infections. Keep in haler clean, monitor drugs effectiveness.

Solumedrol (Methylprednisone) Profilactic, corticosteroid
Definition
Class: Anti-inflammatory, immunosuppressant.
Action: Relieves inflammation and suppresses immune system function.
Side Effects: Euphoria, insomnia, arrhythmias, circulatory collapse, fatal arrest, heart failure, thromboembolism, pancreatitis, acute adrenal insufficiency. Contraindications: Allergy to drug and those with systemic fungal infections.
Nursing considerations: May precipitate Tourette syndrome in children, monitor for excessive stimulation, BP, Check CBC differentail and platelet count with long term use. May delay growth in children, give at least 6 hours before bedtime, after meals to reduce appetite suppresion.

Flovent (Fluticasone) Profilactic, corticosteriod
Definition
Class: Intranasal and inhalation anti inflammatory
Action: Improves breathing ability by reducing inflammation
Side Effects: headache, oral, bronchospasm, candidiasis, pharyngitis, upper respiratory tract infection.
Contraindications: Allergy to drug, as primary therapy to status asthmaticus or other acute episodes of asthma in whom intensive measures are needed.
Nursing considerations: Observe for system effects of corticosteroids, observe postoperatively or at times of stress for evidence of inadequate adrenal response, growth suppression may occur in children with long term use.

Definition
Class: Antiasthmatic
Action: Improves breathing
Side Effects: Headache, dizzy, dental pain, infectious gastroenteritis.
Contraindications: If acute asthmatic attacks or status asthmaticus. If allergy to drug.
Nursing considerations: Monitor for adverse effects.

Zidovudine (Retrovir), antiretroviral
Definition
Class: Antiretroviral
Action: Reduce symptoms of HIV infection
Side Effects: Seizures, agranulocytosis, bone marrow suppression, thrombocytopenia, lactic acidosis, rash, anemia.
Nursing considerations: monitor CBC with differential and platelet counts every 2 weeks. Temporarily decreases risk of illness and death in patients with AIDs and AIDs related complex.

Nelfinavir (Viracept), antiretroviral
Definition
Class: Antiretroviral
Action: Produces immature non infectious virus, prevention of AIDS progression.
Side Effects: Seizures, suicidal ideation, diarrhea, pancreatitis, leukopenia, thrombocytopenia, hepatitis, hypoglycemia.
Contraindications: If receiving amiodarone, ergot derivatives, lovastatin, midazolam, pimozxide, quinidine, simvastatin, or triazolam. Also in children and pregnant women not currently on Viracept therapy due to the presence of EMS, a known carcinogen. Pregnant women should be taken on Viracept.
Nursing considerations: Monitor liver function, assess patients with bleeding tendecies closely, excessive diarrhea, not recommended to mix medication with acidic foods because of bitter taste.

Definition
Class: Antineoplastic, immunosuppressant, antirheumatic.
Action: Kilss certain cancer cells and reduces inflammation.
Side Effects: leukoencephalopathy, intestinal perforation, renal impairment, renal failure, anemia, leukopenia, thromocytopenia, acute toxicity, chronic toxicity, cirrhosis, hepatic bibrosis, pulmonary fibrosis, pulmonary interstitail infiltrates, severe infections, suddnen.
Contraindications: Those with psoriasis or rheumatiod arthritis who also have alcoholism, alcoholic liver, chronic liver disease, immunodeficiency syndromes, or blood dyscrasias.
Nursing considerations: Monitor for effectiveness, get baseline pulmonary function test prior, I and O, monitor uric acid levels, liver function, CBC stay alert for adverse reactions.

Definition
Class: Antineoplastic
Action: kills specific types of cancer cells, improves renal function in mild nephrotic syndrome in children.
Side Effects: cardiotoxicity, anemia leukopenia thrombocytopenia, pulmonary fibrosis, anaphylaxis, secondary malignancies.
Contraindications: Those with severe bone marrow depression.
Nursing considerations: Monitor CBC, renal, and liver functions.

Vincristine, chemotherapeutic
Definition
Class: Antineoplastic
Action: inhibits replication of certain cancer cells.
Side Effects: Coma, permanent neurotoxicity, seizures, phlebitis, constipation, cramps, intestinal necrosis, nausea, stomatitis, vomiting.
Contraindications: Those with demyelinating form of Charcot-Marie-Tooth syndrome. Don't give to patients who are receiving radiation therapy through sites that include the liver.
Nursing considerations: Assess patient before starting therapy, and thereafter, Monitor for acute bronchospasm, hyperuricemia, check for depression of Achilles tendon reflex, numbness, tingling, footdrop or wristdrop, difficulty in walking, ataxia, and slapping gait, ability to walk on heels, fowel function, neurotoxicity.

Monistat antiinfective
Definition
Class: Antifungal
Action: is effective only for candidal vulvovaginitis.
Side Effects: Burning sensation, cramping, headaches, hives, irritation, rash, vulval or vaginal itching.
Contraindications: If allergic
Nursing considerations: If symptoms persist, or if an irritation or allergic reaction develops while you are using Monistat, notify your doctor.
The hydrogenated vegetable oil base of Monistat 3 may interact with the latex in vaginal diaphragms, so concurrent use of these two products is not recommended.

Oxytocin (Pitocin), Hormonal agent and uterine relaxant
Definition
Class: Lactation stimulant, Oxytocic.
Action: Induces labor and milk ejection and reduces postpartum bleeding.
Side Effects: Maternal: Coma from water intoxication, seizures, subarachnoid hemorrhage from hypertention, arrhythmias, hypertension, postpartum hemorrhage, uterin rupture, fatal afibrinogenemia, anaphylaxis.
Fetal: Permanent CNS or brain damage, neonatal seizures, bradycardia, anoxia, asphyxia, fetal death.
Contraindications: Allergy, cephalopelvic disproportion or delivery that requires conversion, fetal distress, prematurity, severe toxemia, hypertonic uterine pattern, total placenta previa, or vasa previa.
Nursing considerations: Assess patient, record contractions and fetal monitoring, VS, I and O.

Methylergonovine (Methergine) Hormonal agent
Definition
Class: Uterine stimulant
Action: Prevention and control of postpartum hemorrhage.
Side Effects: Nausea, vomit, stomach pain, diarrhea, dizzy, chest pain, blurred viion, convulsions, shortness of breath, coldness in your hands and feet.
Contraindications: HTN, toxemia, hypersensitivity to Ergots (fungi).

Dinoprostone (Cervidil), hormonal agent
Definition
Class: Oxytocic, Hormonal agent
Action: used in a pregnant woman to relax the muscles of the cervix (opening of the uterus) in preparation for inducing labor at the end of a pregnancy.
Side Effects: nausea, stomach pain; back pain; or feeling of warmth in the vaginal area. Allergic reactions.
Contraindications: acute pelvic inflammatory disease; active cardiac, pulmonary, renal or hepatic disease. membranes; placenta previa.
Nursing considerations: Uterine activity, fetal status, pregression of cervical dilation and effacement should be monitored. Remove after 12 hours.

Hemabate (Carboprost), hormonal agent
Definition
Class: Oxytocic, hormonal agent.
Action: Carboprost is used to treat severe bleeding after childbirth (postpartum).
Side Effects: ringing in ears, N/V/D, blurred vision, severe headache.
Contraindications: Hypersensitivity to Hemabate Sterile Solution
Acute pelvic inflammatory disease
Patients with active cardiac, pulmonary, renal or hepatic disease.
Nursing considerations: Carboprost is given as an injection into a muscle. You will receive this injection in a clinic or hospital setting. You may also be given medication to control nausea, vomiting, or diarrhea caused by carboprost.

Definition
Class: Gastric mucosal protectant, hormonal agent.
Action: Protect gastric mucosa from ulcerating. Cervical ripening.
Side Effects: Abdominal pain, diarrhea, pancreatitis, anaphylaxis.
Contraindications: with known allergy to prostaglandins and pregnant women.
Nursing considerations: GI history, Negative pregnacy test two weeks prior, don't start therapy until the second or third day of menstrual cycle.

Definition
Class: Analgesic, adjunct to anesthesia
Action: Relieves pain and enhances anesthesia.
Side Effects: sedation, bradycardia, pulmonary edema, respiratory depression
Contraindications: Allergy
Nursing considerations: Assess pain prior and ongoing, watch for drug effectiveness, respiratory distress, constipation, stool softener.

Definition
Class: Analgesic, anesthetic
Action: Relieves pain.
Side Effects: asthenia, clouded sensorium, confusion, euphoria, sedation, somnolence, arrhythmias, bradycardia, apnea, respiratory depresion.
Contraindications: Drug intolerance, Patients who need postoperative pain management have acute mild or intermittent pain that can be managed with nonopioid drugs. Opioid tolerant, increased intracrania pressure, impaired consciousness or coma.
Nursing considerations: Eval. Pain, monitor VS, bladder function, arterial O2 sat, assess for Respiratory depression.

Definition
Class: Vitamin
Action: Helps maintain normal calcium and phosphate levels.
Side Effects: arrhythmias, toxicity. N/V, dry mouth , metallic taste and constipation.
Contraindications: with Hypercalcemia, too much D, renal osteodystrophy with hyperphosphatemia.
Nursing considerations: monitor effectiveness, toxicity, possible malabsorption.

Action: Nutritional supplement
Side Effects: Bronchospasm, nausea.
Contraindications: Underdiagnosed B12 deficiency or underdiagnosed anemia.
Nursing considerations: assess extend of deficiency to folic acid, CBC, monitor throughout, balanced diet.

Class: Analgesic, antipyretic, antiinflammatory
Action: Relieves pain, fever, and inflammation.
Side Effects: dizzy, drowzy, Headache, heart failure, peripheral edema, tinnitus, agranulocytosis, aplastic anemia, leukopenia, neutropenia, pancytopenia, thrombocytopenia, bronchospasm, Stevens-Johnson syndrome.
Contraindications: Allery, nasal polyps, angioedema, and bronchospastic reactions to aspirin or other NSAIDs.
Nursing considerations: Eveluate for relief of pain, renal hepatic function for long term use, Give with meals or mild.

Definition
Class: Heavy Metal Antagonist
Action: Abolishes effects of lead poisoning.
Side Effects: Nephrotoxicity, renal failure, tubular necrosis, hypomagnesia
Contraindications: Allergic, those with anuria, suspected hypocalcemia, renal disease, active or healed tubercular lesions, history of seizures or intracranial lesions.
Nursing considerations: Watch for systemic reaction, monitor effectiveness with calcium levels after each dose, kidney function tests.

Class: Heavy metal agonist
Action: removing a heavy metal, such as lead or mercury, from the blood.
Side Effects: Allergic, fast heart rate, feeling anxious or restless;
pain or tightness in your throat, chest, or hands; burning sensation of your throat, mouth, or lips; or burning sensation in your penis, nausea, vomit, eye twitching,fever sweating, numb or tingling esp. mouth
Nursing considerations: Allergic, monitor liver and kidney, this contains peanut oil.

Action: Inhibits suseptible bacteria
Side Effects: seizures, N/V/D, stomatitis, toxic nephrosis with oliguria and anuria, agranulocytosis, aplastic anemia, leukopenia, thrombocytopenia, hepatic necrosis, epidermal necrolysis, erythema multiform, Stevens-Johnson syndrome, allergic reaction.
Contraindications: with megaloblastic anemia, porphyria, renal impairments, allergic.
Nursing considerations: Allergies, assess infection prior to and ongoing, obtain C&S prior to start of therapy, I and O, Urine output of 1500ml/day.

Contraindications: Allergies to penicillins or this drug.

Nursing considerations: C&S, Hx, Monitor s/s, liver function.

Nursing considerations: Monitor for renal toxicity, dehydration, renal disease, mental condition.

Action: Promotes calmness in hyperkinesis, and prevents sleep.
Side Effects: insomnia, nervousness, seizures, palpitations, tachycardia, abdominal pain, anorexia, decreased appetite, nausea, vomit, leukopenia, thrombocytopenia, thrombocytopenic purpura, erythema multiforme.
Contraindications: Allergic, glaucoma, motor tics, family Hx, Tourette syndrome or Hx of marked anxiety, tension or agitaion, or within 14 days of taking MAO inhibitor.
Nursing considerations: May precipitate Tourettes syndrome in children, Monitor closely, VS, CBC, platelets, differentials, s/s of hematologic toxicity.

Class: CNS Stimulant
Action: Helps prevent sleep and calms hyperactive children.
Side Effects: Insomnia, restlessness, arrhythmias, cardiomyopathy.
Contraindications: allergy, if taken within 14 days of MAO inhibitor, hyperthyroidism, glaucoma, advanced arteriosclerosis, Hx drug abuse.
Nursing considerations: Obtain Hx, monitor growth in children, sleep pattern, excessive stimulation, give at least 6 hours prior to bedtime. Could become dependent with long term use.

Class: Amphetamine
Action: Reduces the effects of ADHD and Narcolepsy
Side Effects: irregular heart beat, increased BP, blurred vision, restlessness, motor tics, headache, dizzy, sleep problmes, dry mouth, diarrhea, weight loss, loss of interest in sex, impotence or difficulty having an orgasm.
Nursing considerations: Take with full glass of water, do not crush, chew, break, or open an extended release capsule.

Action: Hinders fungal growth
Side effects: headache, N/V/D, abdominal pain, hepatotoxicity, anaphylaxis
Contraindications: Hypersensitiver to drug or any of it's components.
Nursing considerations: Assess prior to drug therapy and reassess regularly, periodically monitor liver function, GI and hydration.

Definition
Class: Estrogen, antineoplastic, antiosteoporotic.
Action: Provides estrogen replacement, relieves vasomotro menopausal symptoms and vaginal dryness, helps prevent severity of osteoporosis, and provides palliation for prostate and breast cancer.
Side Effects: Seizures, stroke, MI, pulmonary embolism, thromboembolism, nausea, pancreatitis, endometrial cancer hepatic adenoma, breast cancer.
Contraindications: Those with thrombophlebitis, thromboembolic disorders, extrogen dependent neoplasia, breast or reproductive organ cancer (except for palliative therapy), or undiagnosed abnormal genital bleeding.
Nursing considerations: Monitor, obtain Hx. Causal of endometrial cancer and affects liver function.

Class: Antineoplastic, Estrogen antagonist/blocker.
Action: Hinders function of breast cancer cells.
Side Effects: Stroke, N/V/D, endometrial cancer, uterine sarcoma, leukopenia, thromboytopenia, hepatic necrosis, pulmonary embolism.
Contraindications: allergy, women receiving coumarin-type anticoagulants, Hx of deep vein thrombosis or pulmonary emboli.
Nursing considerations: Assess prior, monitor lipid levels, CBC, calcium, hydration, and GI.

Definition
Class: Anticonvulsant, sedative-hypnotic (barbiturate)
Action: Prevents and stops seizure activity, promotes calmness and sleep.
Side Effects: bradycardia, N/V, apnea, respiratory depression, erythema multiforme, Stevens-Johnson syndrome, angioedema.
Contraindications: allergy, those with hepatic dysfunction, respiratory disease, dyspnea or obstruction, nephritis, or Hx of manifext or latent porphyria.
Nursing considerations: NOT FIRST CHOICE, USUALLY USED IN COMBINATION WITH OTHER ANTICONVULSANTS. Assess prior, then monitor drug levels, CNS, don't abruptly stop, IM injection.

Class: Anticonvulsant, anxiolytic
Action: Prevents or stops seizure activity.
Side Effects: ataxia, behavioral disturbances, suicidal ideation, increased salivation, leukopenia, thrombocytopenia, respiratory depression.
Contraindications: Allergic, have acute angleclosure glaucoma or significan hepatic disease.
Nursing considerations: assess prior to therapy, monitor drug levels, CBC, liver function test.

Diazepam (Valium), Anticonvulsant
Definition
Class: Anxiolytic, skeletal muscle relaxant, anticonvulsant, sedative-hypnotic.
Action: Relieves anxiety, muscle spasms, and seizures, promotes calmness and sleep.
Side Effects: BLOOD DYSCRASIAS, LIVER TOXICITY. ataxia, drowsiness, hangover, lethargy, pain, bradycardia, CV collapse, respiratory depression, acute withdrawal syndrome, phlebitis at injection site.
Contraindications: allergy, angle-closure glaucoma, shock, coma, or acute alcohol intoxication.
Nursing considerations: SEDATION, RAPID WITHDRAWAL IS DANGEROUS. Obtain Hx prior to therapy and reassess after start, monitor liver, kidney, and hematopoietic function studies in patient receiving repeated or porlonged therapy.

Side Effects: BLOOD DYSCRASIAS, HEPATOTOXICITY, GINGIVAL HYPERPLASIA, FOLIC ACID DEFICIENCY. ataxia, confusion, slurred speech, agranulocytosis, leukopenia, pancytopenia, thrombocytopenia, toxic hepatitis, hirsutism, Stevens-Johnson syndrome, toxic epidermal necrolysis, Contraindications: sinus bradycardia, SA block, second or third degree AV block, Adams-Strokes syndrome.
Nursing considerations: DROWSY, INSOMNIA. Assess prior to start, monitor phenytoin level (therapeutic levels), monitor CBC and calcium levels, check vials, blood pressure, and ECG during IV administration. GIVE WITH MILK, MONITOR DRUG LEVELS.

Class: Anticonvulsant, anelgesic
Action: Prevents sizure activity, eliminates pain caused by trigeminal neuralgia, and by an unknown mechanism, treats bipolar disorder.
Side Effects: DROWSY. Worsening of seizures, heart faiure, N/V/D, agranulocytosis, aplastic anemia, thrombocytopenia, hepatitis, erythema multiforme, Stevens-Johnson syndrom, water intoxication.
Contraindications: allergy, previous bone marrow suppression, and within 14 days of using MAO inhibitor therapy.
Nursing considerations: Asses seizure disorder before and monitor during therapy. Obtain basline LABS AND MONITOR DRUG PLASMA LEVELS.
