Trade name: Narcan

Classification: narcotic antagonist, Pregnancy class c

Actions: blocks the effects of both natural and synthetic narcotics and may reverse respiratory depression

Indications: narcotics overdose: morphine, dilaudid, Fentanyl, Demerol, paregoric, methadone, heroin, percodan, tylox. Synthetic narcotic overdose: nubain, Stadol, talwin, darvon. Coma of unknown origin. OD w/ respiratory depression

Contraindications: hypersensitivity, non-narcotic induced respiratory depression

Side effects: N/V, reoccurrence of pain, seizure

Dose/Route: 0.4-2 mg IV, IM, IN repeated in 2-3 min as needed up to 10 mg. Peds 0.01 mg IV, IM, IN repeated in 2-3 min as needed up to 10mg

Onset: within 2 min; Duration: 20-60 min; Half life: 30-60 min

Definition
Trade name: Morphine

Classification: narcotic analgesic, opiate agonist, schedule II, Pregnancy class C

Actions: potent analgesic and sedative that causes some vasodilation reducing venous return and reduced myocardial O2 demand; binds to opiate receptors, causes histamine release.

Indications: moderate to severe pain, MI, pulmonary edema, reducing preload (CHF pt’s)

Contraindications: hypersensitivity to opiates, undiagnosed head or abdominal injury, hypotension, volume depletion, acute bronchial asthma, COPD, severe respiratory depression, pulmonary edema due to chemical inhalation

Side effects: altered LOC, respiratory depression, dizziness, hypersensitivity

Dose/Route: 0.1 mg/kg (2-10 mg standard for an adult) IV, SC, IM

Onset: about 1 min; Duration: 2-4 hours; Half life: 2-3 hours

Definition
Trade name: Versed

Classification: sedative, benzodiazepine, anticonvulsant, Class IV, Pregnancy class D

Actions: short acting benzodiazepine with CNS depressant, muscle relaxant, anticonvulsant, and anterograde amnestic effects

Indications: induce sedation for cardioversion or intubation, acute anxiety states, seizures, stimulant OD (cocaine)

Contraindications: hypersensitivity, narrow angle glaucoma, shock, coma, acute alcohol intoxication

Side effects: drowsiness, hypotension, amnesia, respiratory depression

Dose/Route: 0.1 mg/kg IV, seizure usually 5 mg, sedation usually 1-2.5mg; 0.2 mg/kg IM/IN, seizure usually 10 mg, sedation usually 5 mg. Peds 0.05-0.2 mg/kg IV

Fentanyl
Definition
Trade name: Sublimaze

Classification: narcotic analgesic (opiate agonist), schedule II, Pregnancy class B

Actions: potent synthetic narcotic analgesic similar to morphine and meperidine but with a more rapid and less prolonged action. CNS depressant; Causes endorphin release (feeling of happy)

Indications: severe pain, adjunct to RSI, maintenance of analgesia

Contraindications: MOAI within 14 days, myasthenia gravis, shock, severe hemorrhage, undiagnosed abdominal pain

Side effects: dizziness, LOC/AMS, bradycardia, N/V

Dose/Route: 1 mcg/kg (50-100 standard for an adult) SIVP, 2 mcg/kg IN Peds: 2.0 mcg/kg slow IV

Onset: about 2 min; Duration: 30-60 min; Half life: 2-3 hours
