Pearl: in postop patient, this may be only sign of gastric distension, which can lead to necrosis.  If persistent, KUB and NGT.

 

Differential diagnosis

Esophagitis (chemical, infectious)

 

Drugs

   Metoclopramide

   Sulfa

   Alcohol

   Steroids

   Barbiturates

 

CNS disorder (metastases, encephalitis)

Uremia

Pleural or subdiaphragmatic disease

Phrenic or vagal nerve irritation

Psychogenic

Idiopathic

 

Treatment - largely empiric and paradoxical

   Chlorpromazine (Thorazine) or Haldol

   Valsalva maneuvers

   Lorazepam (Ativan)

   Metoclopramide (Reglan)

Thorazine 10 mg by mouth every 6-8 hours as needed for hiccups.

97017025 39M, sepsis, persistent fevers, persistently positive bacteremia X a few times. So we decided to get ID consult and an echo. Because of his bacteremia, daily blood cultures wre also indicated.  He also had platelts that were trending down each day, to 108 today so we sent for a HIT. He also had bicarb of 16, so we added 60mq Na acetate to his TPN. He also got a CT because of his persitent fever to look for abscess, but they found a severly distended bladder instead with hydroureter and hydronephrosis. He also got Thorazine 25mg for hiccups ovrnight (the pharmacist thought that was too high of a dose). 10/24/11
