Aspirate of a skin mass on the hock of a cat: There is an amorphous dusky purple aggregate of extracellular material (arrow) surrounded by round to spindled cells with elongate to folded nuclei, open chromatin and light purple cytoplasm with indistinct borders (Wright's stain, 500x). In other areas of the aspirate, there were numerous plasma cells (not shown). A cytologic diagnosis of extramedullary plasmacytoma with amyloid deposition (presumptive) was made and was confirmed on histologic examination of a biopsy of the mass. The cells surrounding the presumptive amyloid are likely macrophages versus tumor cells, however smudged plasma cells can resemble histiocytes, in our experience. Macrophages were observed on histologic examination of the biopsy and diffusely infiltrated the mass. |

A few cells also have a tailing appearance to the cytoplasm, but this is considered an artifact of smearing (Wright's stain, 500x). Notice the binucleated cell. Careful inspection of the cells shows that some have indented nuclei. This feature, with the lightly stippled chromatin, somewhat indistinct cytoplasmic boundaries and light cytoplasm are more consistent with a histiocytoma than a plasmacytoma. Unfortunately, no lymphocytes are present in this aspirate to steer us in the direction of a histiocytoma |

Aspirate from a raised red lesion on the eyelid of a dog: There was a mixture of cells, dominated by lymphocytes, with fewer lighter staining histiocytic cells (arrows) and neutrophils (Diff-quik, 500x). The lymphocytes were mostly small cells, although low to moderate numbers of large lymphocytes (L) were also seen. The histiocytes have oval to round to indented nuclei with coarse chromatin and a moderate amount of light cytoplasm, with somewhat indistinct boundaries. One of these cells is mitotic (M). In other areas of the smear, there were also increased numbers of neutrophils, secondary to ulceration of the mass. The results were compatible with a regressing histiocytoma. Although small lymphocytes (cytotoxic T cells) are the expected tumor-infiltrating cell associated with regression, we have seen more and more cases with variable numbers of large lymphocytes as depicted here. When the histiocytes are few in number, it can be difficult to recognize the lesion as a regressing histiocytoma (the physical description helps) and other differential diagnoses include an inflammatory lesion (e.g. secondary to a tick bite) or cutaneous lymphoma (mixed variants are seen rarely in dogs) |
