Irregular fluctuations in baseline FHR of 2-6 cycles per minute (STV—short term variability).
A visually apparent abrupt increase in FHR above the baseline rate of >15 bpm for > 15 seconds. It is called the 15 x 15 rule. For preterm fetuses it is 10 x 10.
Visually apparent gradual decrease in and return to baseline FHR associated with uterine contractions, but begins after the contraction has started and the lowest point of the deceleration occurs after the peak of the contraction. Persistent and repetitive late decelerations over a period of time are ominous and usually indicate the presence of fetal hypoxemia due to insufficient placental perfusion. It can lead to hypoxia and acidemia progressing to acidosis.
A visual abrupt decrease in FHR below baseline usually >15 bpm for at least 15 seconds and usually returns to baseline in less than 2 minutes from the time of onset. Caused by compression of the umbilical cord. Occur at any time.
