What is SBAR?
Definition
SBAR stands for Situation-Background-Assessment-Recommendation. SBAR is a
standardized language technique that provides a structure for hand-offs in patient care responsibility between health care professionals. SBAR application scenarios include:
•nurse-to-physician report on changing patient status;
•nurse-to-nurse report at change of shift; or
•physician-to-physician hand-off transfer to on-call responsibility.

SBAR dialogue begins with a description of the patient’s current situation, provides a brief medical background, leads to a statement of assessment of the patient, and concludes with recommendations for a plan of action.

Definition
Skimming refers to reading through a passage with great precision. The reader will be looking for clues and evidence so as to help him interpret.

Scanning refers to the act of briefly reading through the passage. The purpose of scanning is to just simply know what the passage is talking about, who are the characters, and some other information that is more of general.
