S: Combination of both dorsal and ventral roots, contains 1000's of nerve processes

F: Mixed nerve, provides a two-way pathway for communication between the CNS and PNS

Cervical Plexus (1)

The nerve runs inferiorly through the thorax

Supplies both motor and sensory fibers of the diaphram, innervates breathing movements 

Major end branch of the Lateral Cord,

the nerve runs a litle below the elbow

Muscular Branch - Anterior arm, supplying motor fibers to bicep

Cutaneous Branch - Provides sensation of the lateral forearm

The Lateral Cord branches onto the Medial Cord,

the nerve runs all the way down to the fingers 

Muscular Branch - Anterior Forearm, 5 palm muscles

Cutaneous Branch -  Sensation of the Lateral 2/3 of the hand, and fingers 1 - 4

The Medial Cord, the nerve extends all the way down to the fingers

Muscular Branch - Medial Forearm, intrinsic (essential) muscles of the palm, and finger flexon

Cutaneous Branch -  Sensations of the medial third of the hand, and fingers 4 and 5

The Posterior Cord, the nerve runs posterior to the neck of the humerous

Muscular Branch - Deltoid and shoulder muscle

Cutaneous Branch - Sensation of the shoulder reagion

The Posterior Branch, the nerve wraps around the humerous, runs anteriorly around the lateral portion of the elbow, and continues laterally down the radius to the posterior portion of the hand

Muscular Branch - Innervates extension in the posterior muscles of the arm, forearm, and hand

Cutaneous Branch -  Sensation of the posterolateral surface of the entire limb

Lumbar Plexus (2) 

The nerve enters the thigh and divide into sevaral large branches 

Muscular Branch - Innervates the anterior thigh muscles (Quaudreceps) for thigh flexion and knee extension

Cutaneous Branch - Sensation of the anterior thigh and medial surface of the leg from the knee to foot 

Lumbar Plexus / Obturator Nerve 

The nerve enters the medial thigh through the obturator foramen

Muscular Branch - Innervates adduction muscles

Cutaneous Branch - Sensations from the medial thigh, hip, and knee joints 

Sacral Plexus (1) 

Composed of two nerves,

The Tibial and Common Fibular

they diverge just proximal to the knee

Cutaneous Branches - Sensations from posterior surface of the leg and sole of the foot

Motor (Effector) Branches - Muscles from the back of thigh, leg, and foot

Cutaneous Branches - Sensations from the anterior surface of the leg and dorsum of the foot

Motor Branches - Extension of the biceps femoral of the thigh, fibular muscles of the lateral compartment of the leg, tibialis anterior, and toes
