CLASS: Sympathetic Agonist
ACTION: Epinephrine is a naturallyoccuring CATECHOLAMINE. It potent Alpha & Beta adrenergic stimulants. More effect on Beta Receptos.
CARDIO ACTION: Increase in Heart Rate, Cardio Contractile Force, Electrical Activity in the myocardium, Systemic vascular resistance, Blood Preasure, Automaticity. Converts fine V-Fib to Course V-Fib
INDICATIONS: Cardiac Arrest, Asystole, V-Fib, Pulseless V-Tach, PEA, Bradycardia, Hypotention, Anaphylaxis, Reactive Airway disease
CONTRAINDICTIONS: 1:10000 is not for patients who do not require Cardiopulmonary resuscitation efforts.
SIDE EFFECTS: Palpatations, Anexiety, Headache, Increased in myocardial oxygen demand, maybe inactived by alkaline solutions like Bicarb.
PRECAUTIONS: Raise in BP, Increase in HR, Angina, and Increase in myocardial oxygen demand. High Dose do not improve survival or neurological outcome. Higher doses may be required to treat poison/drug induced shock.
ROUTE/DOSE: IV: 1.0mg (10ml of 1:10000 sol) every 3-5 followed by 20ml IV Flush ETT: 2.0-2.5mg 1:10000 diluted in 10ml Normal Saline every 3-5 min
CHILD DOSE: IV/IO 0.01 mg/kg (0.1ml/kg) of a 1:1000 sol. Second and subsequent dose is 10 times the intital dose. The total volume of drug adminstered. ETT: 0.1mg/kg (0.1ml/kg) of a 1:1000 solution. Every 3-5 min |

CLASS: Sympathetic Agent
ACTION: Dopamine is a naturally occuring CATECHOLAMINE. It acts on Alpha and Beta 1 and Dopaminergic adrenergic receptors. LOW DOSE: Renal Vasodilation (Dopaminergic effects) MED DOSE: Beta effects, vasoconstriction HIGH DOSE: Alpha effects, excessive vasoconstriction.
INDICATIONS: Symptomatic bradycardia refractory to atropine Cardiogenic Shock Low urinary output CHF
SIDE EFFECTS: Ectopy (SVT and Ventrical Arrhythmias) Tachycardia, N/V, Headache, Peripheral vasoconstriction
PRECAUTIONS: Taper slowly, Do not mix with Bicarb
DOSE/ROUTE: Mix 400 mg in 250 ml of D5W or 800mcg in 500 ml D5W (1600 mcg/ml) 1-5 mcg/kg/min= Dopaminergic effects 5-10mcg/kg/min=Beta Effects (Cardiac Dose) 10-20mcg/kg/min=Alpha Effects (Vasopressor dose) >20mcg/kg/min= Alpha & Beta effects |

CLASS: Hormones and hormone modifiers
ACTION: Stimulates the contraction of smooth muscle in coronary, GI and muscular vascular beds. Effect of contraction most prominet in the capillaries, small arterioles and venules With less effect on smooth muscular of large veins Exogenous parental form of antidiuretic homone (ADH)
INDICATIONS: Maybe used as an alternative pressor to epi in the treatment of adult shock refractory to VF
CONTRAINDICATIONS: Not recommended for responsive pts with Coronary artery disease. Rapid increase in extracellular fluid could be hazardous to these patients. Angina, Arteriolerosis, Asthma, Chronic nephritis
SIDE EFFECTS: Potent peripheral vasoconstrictor Increased peripheral vascular resistance may provoke cardiac ischemia and angina
DOSE: 40 units one time for IV, IO, ETT |
