Origin at iliac fossa and os coxa/ hip bone. Insertion attachment at lesser trochanter, medial to greater trochanter.

Supplied by femoral nerve - L2-3

Origin at bodies and bases of transverse processes of the spine L1 to L5. Insertion at lesser trochanter of femur, medial to greater trochanter - overlapping iliacus muscle.

Supplied by ventral rami of lumbar plexus - L1-L3

Origin attachment to anterior iliac crest. Insertion attachment to condyle of fibula - (via iliotibial band).

Supplied by superior gluteal nerve - L4-5, S1

Origin attachment at anterior superior iliac spine and region slightly below it. Insertion at proximal tibia, distal to femur - medial to tibial tuberosity. 

Supplied by femoral nerve - L2-3

Origin attachment at anterior inferior iliac spine and ilium above the acetabelum. Insertion attachment at quadriceps tendon to base of patella and also to tibial tuberosity via patellar ligament.

Supplied by femoral nerve - L2-L4.

Origin attachment at pectineal line along superior ramus of pubis. Insertion to medial posterior surface of femur, inferior to lesser trochanter.

Supplied by femoral nerve - L2-L4

Origin attachment at superior aspect of pubis, below pubic tubercle. Insertion along medial lip of femur at middle third of linea aspera.

Supplied by obturator nerve - L3-4

Origin at lateral surface of inferior ramus of pubis. Insertion at proximal aspect of linea aspera. 

Supplied by obturator nerve - L3-4

Separated in to two heads:

Vertical head: Origin attachment at ischial tuberosity. Insertion at adductor tubercle of femur - Supplied by sciatic nerve.

Oblique head: Origin attachment at inferior ramus of pubis & ischial ramus. Insertion attachment at glueteal tuberosity, linea aspera and proximal supracondylar line of femur. - Supplied by obturator nerve (L2-4)

Gracilis 

Origin attachment at inferior border of pubic body near the symphysis. Insertion attachment a proximal medial surface of tibia.

Supplied by obturator nerve - L2-L4

Origin attachment on surface of ilium posterior to gluteal line and posterior inferior surface of sacrum and coccyx. Insertion attachment at gluteal tuberosity of femur, anterior aspect of lateral tibial condyle via the iliotibial band (tract). 

Supplied by gluteal nerve - L5, S1-S2
