Infiltration-Maxillary

a) Dental plexus of the injected site.

b) At the height of the mucobuccal fold closest to the apex of the tooth to be anesthetized.

c) A bony ridge, or eminence, can usually be palpated in the maxillary teeth.  This serves as a landmark for visualizing the long axis of tooth and locating the apex of the tooth for penetration site.

d) 27 or 25 guage short needle.

e) Align barrel of the syringe parallel to long axis of the tooth, following the contour of the maxilla.

f) 3-6mm

g) 0.6 ml (1/3 of a cartridge).

Maxillary-PSA

a) Teeth anesthetized: maxillary molars except mesiobuccal root of first molar.

Periodontium/soft tissues: buccal to affected teeth.

b) At the height of mucobuccal fold, posterior to the zygomatic process of the maxilla and generally superior to the distobuccal root of the maxillary second molar.

c) Posterior to zygomatic process of maxilla and superior to distobuccal root of maxillary second molar.

d) 25 or 27 gauge usually a short needle is recommended.

e) Angle of the neddle insertions is upward (at a 45 degree angle to the occlusal plane of maxillary teeth), inward behind maxillary tuberosity (at a 45 degree angle to the midsagittal plane), and then must be advanced backward behind the posterior aspect of the maxilla.  The barrel of the syringe must be angled downward from the occlusal table and outward from midsagittal plane.

f) 16mm (9mm from the hub of an average 25mm short needle.  Can range form 10-16mm for anatomical variances.

g) 0.9ml-1.8ml (1/2 to 1 cartridge) after negative aspiration.

a) Teeth anesthetized: maxillary premolars and mesiobuccal root of first molar.

Periodontium/soft tissues: facial to affected teeth.

b) At the height of the mucobuccal fold over the maxillary second premolar.

c) Mucobuccal fold over maxillary second premolar.

d) 27 or 25 gauge short needle.

e) Follow the contour of the maxilla.

f) 5-8mm

g) 0.9-1.2ml (1/2 -2/3 of a cartridge).

a) Teeth anesthitized: canine, lateral, central.

Periodontium/soft tissues: facial to affected teeth.

b) The height of mucobuccal fold anterior of canine eminence.

c) Above apical area of canine at height of canine fossa.

d) 27 gauge short needle is most common.

e) Align barrel of the syringe parallel to the long axis of canine, follow the contour of the maxilla.

f) Based on location of apex of the tooth and usually within 3-6mm.

g) 0.9ml (1/2 of a cartridge).

a) Teeth anesthetized: none

Periodontium: palatal tissues of posterior teeth.

b) Palatal soft tissue slightly anterior to the greater palatine foramen, at the anterior border of the depression formed by the foramen.

c) Anterior border of the depression formed by the foramen.

d) 27 gauge short needle.

e) Perpendicular to palatal bone at the foramen, with the syringe barrel near the lower lip.

f) Advances slowly 4-10mm through dense mucosal tissue to make gentle contact with bone.

g) Minimum of 0.45ml (1/4 of a cartridge).

Maxillary-PASA

a) Teeth anesthetized: none

Periodontium: palatal to incisors and canines.

b) Palatal mucosa lateral to the widest anteroposterior dimension of the incisive papilla.

c) Incisive papilla.

d) 27 gauge short needle.

e) Lateral to and at the base of the broadest aspect of the incisive papilla.  Continue penetrating at a 45 degree angle under papilla.

f) 4-7mm

g) 0.4ml (little less than 1/4 of a cartridge).

a) Anesthetized: buccal nerve and soft tissue buccal to the mandibular molars.

b) Contact bone.

c) Mandibular molars, retromolar pad, and vestibule.

d) Long needle.

e) Parallel to occlusal plane on side of injection.

f) Contact bone.

g) 1/8 of a carpule.

Mandibular-IA

a) Teeth anesthetized: all teeth in quadrant.

Periodontium/soft tissues: all peridontium, buccal mucosa premolars to midline, floor of mouth and 1/2 tongue in quadrant.

b) Slighlty lateral to the pterygomandibular raphe at a height that is 2 to 3 millimeters superior to the greatest concavity of the coronoid notch, and well medial of the internal oblique ridge.

c) Pterygomandibular raphe, the coronoid notch on the anterior border of the ramus of the mandible, and the internal oblique ridge on the medial surface of the mandible close to the molars and continuing posteriorly.

d) 25 gauge long needle.

e) The syringe barrel is positioned at the labial commisure over premolars on the contralateral side of the mouth. The barrel remains parallel to and above the occlusal plane of mandibular molars throughout injection.

f) After needle contact with bone, withdrawal 1mm.

g) Minimum of 1.5ml (3/4 of a cartridge).

Mandibular-Mental

a) Teeth anesthetized: pulpal limited to tooth at site of infiltration.

Periodontium/soft tissues: premolars to midline.

b) Penetration site varies with location of mental foramen. It's in the depth of the mucobuccal fold superior to the foramen.

c) Mucobuccal fold superior to the foramen.

d) 27 or 25 gauge needle.

e) Syringe is aligned vertically with the patient's cheek to approach the penetration site. Following penetration, advance needle tip at an angle directly vertical to the foramen to a depth just superior to it.

f) 4-6mm

g) Minimum of 0.6ml (1/3 of a cartridge).
