Term
ASIS
Definition
In front of patient place your hands on the sides of his waist with your thumbs on the ASIS and your fingers on the anterior portion of the iliac crests.
Term
Iliac Crests
Definition
Subcutaneous and serves as origin or insertion point for various muscles none of which cross the bony liner crest so it remains available for palpation.
Term
Greater Trochanter
Definition
With your thumbs on the ASIS move your fingers down from the iliac tubercles to the greater trochanters. The posterior edge is relatively uncovered and easily palpated. OR with your thumbs on the PSIS move fingers down to posterior greater trochanters.
Term
Pubic Tubercles
Definition
With your fingers on the trochanters move your thumbs along the inguinal creases medially and obliquely downward to the pubic tubercles. They are the same level as the top of the greater trochanters.
Term
PSIS
Definition
Lie directly under the visible dimples just above the buttocks.
Term
Ischial Tuberosity
Definition
Located in middle of buttock at level of gluteal fold. With fingers on greater trochanter move thumb from the PSIS to the ischial tuberosity. Easier to palpate with hip flexed. In same horizontal plane as lesser trochanters.
Term
Inguinal Ligament
Definition
Located between ASIS and pubic tubercles. Any unusual bulges may indicate inguinal hernia.
Term
Femoral Artery
Definition
Passes under inguinal ligament at it's mid-point. Pulse is palpable just inferior to inguinal ligament halfway between ASIS and pubic tubercle.
Term
Trochanteric Bursa
Definition
Palpate the trochanter for tenderness that might indicate bursitis. The bursa may not be palpable unless it is distended or inflamed - if is the area around it feels boggy and may be tender.
Term
Gluteus Maximus
Definition
Origin and insertion are difficult to palpate. An imaginary line between the coccyx to the ischial tuberosity represents the maximus' lower border, from the PSIS to just above the greater trochanter represents the upper border, and the PSIS to the coccyx represents the medial border. Most prominent in prone with buttocks squeezed together OR prone with hip extended and knee flexed. Should be palpated bilaterally at same time to provide instant comparison of tone, size, shape, quality.
Term
Hamstrings
Definition
To palpate common origins on ischium have patient in sidelying with knees to chest.
