Definition
Chelating agent for use in mercury, arsenic, and lead poisoning. - IM admin in peanut oil sol'n (NB: peanut allergies, fat embolism if IV injection)
- Chelates heavy metals and excreted by kidneys
- Use w/ CaNa2EDTA for acute lead poisoning
- Adverse: N/V, pain at injection site, fever

THE iron chelator - Tx: Acute Fe overload - IV fluids, whole bowel irrigation, DFO
- High affinity binding to free iron
- Fe chelate is excreted in urine (pink pee)
- Heme Fe is unaffected - few side effects
- Adverse: Anaphylactiod rxn: ARDS, HTN w/ rapid IV infusion, bacterial overgrowth of yersinia in prolonged use

Cyanide (d/t smoke inhalation) binds cytochrome Fe and poisons oxid phosphyl'n. Anaerobic respir'n produces lactic acidosis (N/V, seizures, 'rapid knock down') - Cyanide antidote kit - amyl nitrate form MetHB and extracts CN- from cytochrome. Na Thiosulfate detoxifies MetHb + CN is excreted in urine. Adverse: vasodil'n (shock), MetHB toxocity
- Hydroxycobalamine (B12)- competes w/ cytochrome for CN- binding. B12/CN chelate excreted in urine. Synergy w/ Na Thiosulfate - admin@ diff't sites or they fuck eachother up. Adverse: HTN (d/t dcrsd NO)
