Cards

Term
allopurinol
Definition
Prodrug inhibitor uric acid formation via xanthine oxidase (converted by enzyme it inhibits)

SE: rash, nausea, nephritis, cataracts, hepatoxic

used during tumor lysis syndrome

may precipitate crisis so put on colchicine 1st.

CONTRAINDICATED PROBENECID (increases clearance)
Term
colchicine
Definition
anti-inflammatory pathopneumonic for gout

inhibits tubulin polymerization:
anti-migration
anti-LTB4 synthiesis

Has a very poor therapeutic index: GI distress and bone marrow supression

effectiveness dose dependent during attacks
Term
Probenecid
Definition
Increases rate of uric acid secretion by blocking proximal tubule resorbtion

May exacerbate attack!

Increased risk of renal stones and should include nsaids due to initial increase attacks

caution with peptic ulcer disease

inhibited by aspirin
additive salicates
Term
Febuxostat
Definition
Xanthine Oxidase Inhibitor
TX Gout patients intolerant to allopurinol

SE: Rash, GI upset, cataracts, nephritis, hepatotoxic

Reduces urate level better than allopurinol but no more effective on number of attacks

more cardiotoxic
Term
NSAIDS
Definition
COX inhibitors first line for gouty attacks

INFOMETHACIN/NAPROXEN best
NO ASPIRIN/SALICILATE

oxaprozin lowers serum levels and increases possibility stones!
Term
Pegloticase
Definition
recombinant porcine uricase converts uric acid to allatoin

given IV
anaphylaxis approx 25%
horrible gout (3% given + Immunodepressants)
