Anticholinergics

Definition

Artropine (Donatel), Propantheline (ProBanthine), Metantheline Bromide

  • Mech: Dcrs gastric acid production by blocking parasympathetic signals to parietal cells 
  • Adverse: Vagotomy effect: dry eyes/mouth, blurred vision, atony of bladder/intestine (constipation), flushing.
  • Contraindic'd in case of pyloric stenosis, hiatal hernia

H2 Receptor Antagonists
Definition

Ranitidine (Zantac), Cimetidine (Tagamet),

Nizatidine (Axid), Famotidine (Pepsid)

  • Mech: Dcrs gastric acid prod'n by blocking histamine rcptr on parietal cells 
  • Prophylactic/long-term use to prevent ulcers
  • Adverse: Uncommon HA, lethargy, Cytochrome p450 interaction (Cimetidine<Ranitidine) =affects warfarin, dilantin, lidocaine metabolism
  • REBOUND HYPERACIDITY - discontinue slowly

H+/K+ATPase Inhibitors

(PPI's)
Definition

Omeprazole, Pantoprazole, Rabeprazole, Lansoprazole, Esomeprazole

  • Mech: Dcrs acid secretion by blocking H+ pump
  • Slower onset, but more acid inhibition than H2 Rcptr antags
  • Adverse: HA, gynecomastia, p450 inhibition (dcrsd clearance of warfarin, diazepam, dilantin)
  • No long-term treatment: gastric carcinoma in animals
