varicocele
Term
Asymptomatic varicoceles may be seen in up to ______
Definition
10% of the population
Term
where do varicocele's most commonly occur?
Definition
left side (85-95%) because left spermatic vein enters the left renal vein @ a 90 degree angle
Term
MC surgically correctable cause of male infertility
Definition

varicocele

(seen in 30% of infertile men b/c the increased temperature from the increased venous blood flow inhibits spermatogenesis)

Term
Clinical manifestations of varicocele
Definition

"bag of worms" superior to testicle

dilation usually decreases when pt is supine or w/ testicular elevation

usually painless but +- cause a dull ache or heavy sensatiion

Term
Dilation worse when patient is upright or with valsalva
Definition
varicocele
Term
Treatment of varicocele
Definition

 surgery in some cases

Term
sudden onset of left-sided varicocele in older man is......
Definition
possible renal cell carcinoma
Term
R. sided varicocele in children < 10 is possible
Definition
retroperitoneal malignancy
Term
Prostate cancer is usually a slow growing tumor
Definition
most patients die with prostate cancer than from it
Term
Risk factors for prostate cancer
Definition

genetics

diet: high fat intake

obesity

African americans

Adenoma > 95%

Term
Clinical manifestations of prostate cancer
Definition

1. often asx until invasion of bladder, urethral obstruction, or bone involvement

2. urethral obstruction: increased urinary frequency, urgency, decreased urinary stream, urinary retention

3. back pain/bone pain

Term
Prostate Ca assoc w/ increased incidence of.....
Definition

METs to bone, weight loss

(bone/back pain)

Term
when is screening usually done for prostate cancer?
Definition

via DRE & PSA done if >= 59

African-Americans or family hx >= 40

Term
How do you diagnose prostate cancer?
Definition

1. increased prostate specific antigen (PSA)

2. DRE: hard, nodular, enlarged asymmetrical prostate

3. U/S w/ needle bx. if PSA >4. If >10 then bone scan to r/o METS. Gleason grading

Term
PSA > _____ indicates prostate cancer
Definition

10

increased likelihood for prostate cancer & METS

Term
Treatment of local prostate cancer
Definition

radical prostatectomy

+- active surveillance if low grade

S/E: incontinence & impotence

Term
Treatment of advanced prostate CA
Definition

external beam radiation therapy, androgen deprivation (orchiectomy + GnRH agonists)

if bone pain/METS --> localized XRT, crytherapy

Term
autosomal dominant disorder d/t mutations of either genes PKD1 or PKD2
Definition
polycystic kidney disease
Term
multisystemic progressive d/o characterized by formation & enlargement of kidney cysts in other organs (liver, spleen, pancreas)
Definition

polycystic kidney disease

vasopressin stimulates cytogenesis --> ESRD over time

Term
Clinical manifestations of polycystic kidney disease
Definition
  1. abdominal/flank pain: may be d/t infection, nephroliasis, or bleeding into cyst.
  2. palpable flank mass: large palpable kidneys on PE
  3. HTN, hepatomegaly, hematuria, decreased urine concentrating ability, microalbuminuria
  4. extrarenal: cerebral "berry" aneurysms, hepatic cysts, mitral valve prolapse, colonic diverticula
Term
How do you diagnose polycystic kidney disease
Definition
  1. renal u/s : most widely used test. genetic testing also
  2. CT scan: MRI : more sensitive than renal U/S
Term
How do you treat polycystic kidney disease
Definition
  1. simple cyst: observation, periodic reevaluation
  2. multiple: supportive, increased fluid intake - fluids decreases vasopressin (less cytogenesis), control HTN. +- need dialysis or renal transplant
Term
