Azathioprine
Definition
Immunomodulator; Cytotoxic drug. Oral or IV. In body, metabolized to 6-MP, which blocks purine synthesis. Some metabolites are also fake nucleotides that are incoorporated into DNA. Use: with glucocorticoids to block transplant rejection, rheumatoid arthritis and lupus, excessive inflammation (Wegener's granulomatosis and polyarteritis nodosa). blocks both B and T cell immunity, and blocks the inductibe stage of immunity. Toxicities: GI, leukopenia, increased infection and malignancy.

Cyclophosphamide
Definition
Immunomodulator; Cytotoxic agent. Alkylating agent related to nitrogen mustard. Cross links DNA. Blocks inductive stage of immunity, affects B cells and T supressor cells. Use: imflam (Wegeners, nephrotic syndrome, polyarteritis nodosa, lupus). Tox: Hemorragic cystitis (bladder inflam).

Methotrexate
Definition
Immunomodulator; cytotoxic drug. Dihydrofolate reductase inhibitor, bloks DNA synth. At LOW DOSE, one pill once a week, is anti-inflammatory, (impaired neutrophils?). Use: inflam diseases like rheumatoid arthritis, psoriasis, inflam bowel disease (Crohn's disease). Tox: GI, allergic pneumonitis, bone marrow supression, hepatotoxicity***. Check liver function reguarly.

Cyclosporine
Definition
Immunomodulator; T cell specific drug. Revolutionized transplant therapy! Acts only on helper T cells, spares rest of immune system. Mech: binds cyclophilins, blocking generation of calcineurin, blocking DNA synthesis. Also may bind directly to nuclear targets to enhance lymphokine production. Decreases IL-2 production (that activates T cells) and gamma interferon (macrophages). Use: Tissue transplant, hypersensitivity diseases. Tox: Nephrotoxicity (35%), HTN. Low serum cholesterol increases toxicity, so monitor blood. No secondary infection or bone marrow tox except reactivation of EB and lymphoma. Dont use in renal transplants.

Definition
Immunomodulator; T cell specific drug. Decreases T cell synth of IL2, 100 times more potent than cyclosporine.Inhibitis calcineurin phosphate. Side effects similar to cyclosporine, but more headache and neuro probs. P450 metabolism. Use: transplant rejection can use for kidneys, and topical prep for atopic dermititis.

Immunomodulator; humanized monoclonal antibody therapy. Chimeric antibody against TNF-a. Use: Crohn's Disease, rheumatoid arthritis. Tox: increased infections (TB), lymphoma, MS exacerbated.

Immunomodulator; humanized monoclonal antibody therapy. 99% human antibody against IgE. Given subcutaneously every 2-4 weeks for asthma.

Definition
Immunomodulator; humanized monoclonal antibody therapy. Antagonist of TNF. Has 2 TNF binding region and an Fc region, which bings to a phagocytic cell leading to uptake and degredation of TNf. UseL Rheumatoid arthritis. Tox: infections, death from sepsis.

Immunopomodulator/potentiator. Given IV every 4 weeks. Passivly immunize ppl who lack immunoglobulins or who have circulating immune complexes. Tox: anaphylaxis, careful in patients with congenitally deficient IgA, give only types without IgA.

Immunomodilator/potentiator. Mycobacterium closely related to TB. Use: vaccine against TB (potent stimulator of macrophages and T cells) and Tx for bladder cancer (erradicates tumor cells).

Immunomodulator/potentiator. A T cell growth factor. Use: renal cell carcinoma. Tox: serious! hypothyroidism, capillary leak syndrome leading to fatal pulmonary edema.

Immunomodulator/potentiator. Comes from leukeocytes naturally. Use: hairy cell leukemia, leukemias and lymphomas, chronic viral conditions: venereal warts, HepB+C, Karposi's sarcoma in AIDS. Tox: autoimmune thyroiditis and hepatitis.

Immunomodulator/potentiator. Comes from fibroblasts normally. Injections every other day. Anti-viral and T cell reglator. Use: Multiple Sclerosis.

Immunomodulator/potentiator. Made by activated T cells, induces macrophage activation. Use: in diseases with decreased macrophage killing ability: chronic granulomatous disease and visceral leishmaniasis.
