Definition
Dilantin
Use: Partial and Generalized Seizures
PK: Saturable Metabolism. Cyp 3A4 Inducer**
MOA: Enhances sodium channel inactivation.
SE: Nystagmus, Ataxia, gingival hyperplasia, osteomalacia
Clinical Pearls: Make small dosage adjustments due to saturable metabolism and potential for accumulation. |

Definition
Tegretol, Carbatrol, Equetro
Use: Partial > Generalized Seizures
PK: Cyp 3A4 Inducer/Auto-Induction
MOA: Enhancer sodium channel inactivation
SE: Dizziness, diplopia, leukopenia, osteomalacia
Clinical Pearls: May worsen some primary generalized seizure types, especially absence.
Use outside of Epilepsy: Bipolor disorder (good mood stabalizer), Trigeminal neuralgia. |

|
Definition
Trileptal, Oxtellar
Use: Partial > Generalized Seizures
PK: Can be inhibited or induced**
Inhibits minor pathway of Phenytoin, can induce oral contraceptives when given >1200 mg/day
MOA: Enhances sodium channel inactivation
SE: Dizziness, diplopia, ataxia, hyponatremia
Clinical Pearls:Fewer side effects when compared to Carbamazepine. Monitor Sodium levels. |

Vimpat
Use: Partial Seizures
PK: No drug interactions, Cyp3A4 substrate
MOA: Enhance sodium channel inactivation
SE: Dizziness, diplopia,headache, nausea
Clinical Pearls: IV formulation |

Definition
Lamictal
Use: Partial and Generalized Seizures
PK: Can be induced or inhibited, estrogen in OC/pregnancy can increase clearance.
MOA: Enhance sodium channel inactivation.
SE: Sedation, diplopia, ataxia, nausea, rash
Clinical Pearls: When used with inhibitor (Valproic Acid) half life doubles to ~ 60 hours. When used with inducer (Carbamazepine) half-life is cut in half to ~ 15 hours.
Use outside of epilepsy : Bipolar - good mood stabalizer. |

Definition
Topamax, Qudexy XR, Trokendi
Use: Partial and generalized seizures.
PK: Can be induced, can induce oral contraceptive metabolism when dosed > 200mg/day.
MOA: Enhance sodium channel inactivation.
SE: Difficulty concentrating/remembering words, kidney stones, weight loss.
Clinical Pearls: increase fluid intake, watch for use in women of childbearing age taking for headaches while on contraceptives.
Use outside of epilepsy: migraine prophylaxis, weight loss (Qsymia) |
