Term
levothyroxine
Definition
Uses: hypothyroidism
MOAs: body can activate it when necessary (T4)
Adverse Effects: thyrotoxicosis (tachy, angina, tremor heat intolerance), and a-fib and bone loss in the elderly
Drug Interactions: anti-heartburn meds reduce absorption, sedatives/mood stabilizers accelerate absorption
NIs: must be taken on an empty stomach to be properly absorbed
Term
propylthiouracil
Definition
Uses: hyperthyroidism
MOAs: prevents use of iodine in thyroid hormone synthesis
Adverse Effects: menstruation changes, rash, itch, bruise, bleed, swell, goiter, N/V
Drug Interactions: warfarin, digoxin, and theophylline are increased; beta blockers are decreased
NIs: monitor ASTs and ALTs for liver injury; shouldn't be taken during pregnancy or with liver problems; may take weeks to start working
Term
methimazole (Tapazole)
Definition
Uses: hyperthyroidism, graves' disease
MOAs: suppression of thyroid hormone synthesis
Adverse Effects: agranulocytosis, hypothyroidism, cretinism
NIs: must monitor WBC for agranulocytosis; PO;
Term
beta adrenergic antagonists
Definition
Uses: HYPERTHYROIDISM MGMT, htn, anxiety
MOAs: Beta-1 adrenergic antagonism reduces heart rate--helpful in thyroid storm
Adverse Effects: bradycardia, lethargy, CHF, hypotension, & depression
NIs:
Term
radioactive iodine-131
Definition
Uses: hyperthyroidism
MOAs: destroys thyroid tissue w/ gamma and beta rays, full effect after 7 half-lives
Adverse Effects: hypothyroidism
NIs: PO
Term
Lugol's solution
Definition
Uses: hyperthyroidism
MOAs: "non-radioactive iodine" uses feedback mechanism to suppress T3 & T4 production
Adverse Effects: Iodism (oroesophageal pain/discomfort, skin eruptions)
NIs: 2-3 drops TID x 10d in juice to mask the flavor
Term
vasopressin
Definition
Uses: Hypothalamic Diabetes Insipidus
MOAs: ADH (keep water), 2-8 hr duration
Adverse Effects: water intoxication, injury r/t vasoconstriction (angina, MI)
NIs: IM, SubQ; monitor: I&Os, creatinine, electrolytes
Term
desmopressin
Definition
Uses: hypothalamic DI, nocturnal enuresis
MOAs: renal water retention, 8-20 hr
Adverse Effects: water intoxication
NIs: IN, IV, SubQ, PO; monitor: I&Os, creatinine, electrolytes
Term
sandostatin (octreotide)
Definition
Uses: acromegaly, Gigantism
MOAs: Blocks GH release
Adverse Effects: N/D, Gallstones
NIs: lifelong; parenteral
Term
somatropin (Humatrope)
Definition
Uses: GH deficiency
MOAs: GH agonist
Adverse Effects: hyperglycemia
NIs: no effect after closure of epiphyseal plate, monitor ht&wt, monitor for hyperglycemia
Term
hydrocortisone
Definition
Uses: Addison's Disease/adrenal insufficiency; also, inflammation, allergic reactions, and cancer
MOAs: cortisol
Adverse Effects: adrenal suppression, Cushing's Syndrome
NIs: PO, IV, IM; lifelong replacement therapy
Term
fludrocortisones
Definition
Uses: Adrenal insufficiency, hypoaldosteronism
MOAs: mineralocorticoid replacement (salt retention)
Adverse Effects: edema, HTN, cardiomegaly, and hypokalemia
NIs: PO; monitor: weight gain, BP, electrolytes
Term
Cushing's Syndrome
Definition
Adrenal cortex overactivity makes you full and sweet:
obesity, edema, HTN
&
hyperglycemia, glycosuria

removal and replacement to treat, Ketaconazole can block glucocorticoid synthesis short term
