Candida- White cheese-like→ pseudohyphae (KOH)→ Monistat • Rx: Diflucan 100mg tab x 1, if abx recommend daily yogurt or lactobacillis
BV- fishlike vag. Odor= white-gray discharge→ clue cells (normal saline)+Whiff • Rx: Flagyl 500 mg BID x 7 days or 2 grams
Trich- frothy, green discharge, stinky, pruritic, reddened vaginal area, dysuria, strawberry cervix→flagella -> trichomonads (Normal saline) • Rx: Flagyl 2g PO x 1, treat partner or 500 mg bid x 7 days **only one that is considered sexually transmitted |

- May use low dose combined hormone therapy; if contraindicated, may use low dose SSRI/SNRI and/or gabapentin - estrogen (premarin), progestin (not necessary if hysterectomy) - Topical estrogen may be used for atrophic vaginitis

Tx: Penicillin G 2.4 million units IM (primary, secondary, early under 1 yr) - Penicillin G 2.4 million IM x 3 weeks (tertiary or latent) - if allergic, doxy 100 mg BID, erythromycin 500 mg QID |
