Aspirin

Definition

Class: platelet inhibitor, anti-inflamitory agent

Indications: New onset chest pain suggestive of acute MI, signs and symptoms of recent CVA

Contraindications: Tachycardia, hypersensitivity, active ulcer, asthma

Side Effects: Heartburn, GI bleeding, nausea, vomiting, wheezing in allergic pts.

Dosage: 160 mg to 325 mg (chewed if possible) MAX 325 mg Pediatric: not recommended

Amiodarone (Cordarone, Pacerone)

Definition

Class:Antiarrrhythmic

Indications:V-fib/pulseless V-tach and unstable V-tach in pts refractory to other therapy

Contraindications: Hypersensitivity, cardiogenic shock, sinus bradycardia, 2nd or 3rd degree AV block (unless functional pacemaker available)

Dosage: Adult: V-fib/pulseless V-tach unresponsive to CPR defib and vasopressors: 300mg IV/IO (in 20-30mL D5W) initial dose followed by 1 time 3-5 minutes  150mg IV/IO Pediatric: 5mg/kg can repeat up to total dose of 15mg/kg

Oxygen

Definition

Class: Naturally occouring atmospheric gas

Indications: confirmed or suspected hypoxemia, ischemic chest pain, respiratory insuffiency, confirmed or suspected CO poisoning, decreased LOC

Contraindications: certain pts w/COPD or emphysemia intolerable O2 >35% hyperventiliation

Side Effects: Decreased LOC and respiratory depression in pts w/chronic CO2 retention Retrolental fibroplasia (high concentrations) to infants (maintian  30-40% O2)

Dosage:  Adult and pediatric cardiac arrest and CO poisoning 100% Hypoxemia 10-15 L/min via NRBM, COPD 1-6 L/min via NC

Epinepherine (Adrenalin)

Definition

Class: Sympathomimetic

Indications: Cardiac arrest, bradycardia, anaphylaxis

Contraindications: Hypertension, hypothermia, pulmonary edema, myocardial ischemia hypovolemic shock

Side Effects: Hypertension, tachycardia, arrhythmias, pulmonary edema, nausea, headache

Dosage: Adult: mild allergic 0.3-0.5mg, Anaphylaxis 0.1mg,cardiac arrest 1mg every 3-5 min Pediatric:allergic reactions asthma bradycardia0.01mg/kg, cardiacarrest 0.01mg/kg every 3-5min

Atropine Sulfate

Definition

Class: anticholinergic agent

Indications: Hemodynamically unstable bradycardia, asystole, pulseless electrical activity (PEA)

Contraindications: Tachycardia, hypersensitivity, myocardial ischemia, narrow angle glaucoma

Side Effects: Headache, dizziness, nausea, vomiting, tachycardia, arrhythmias

Dose: Adult asystole or bradycardiac PEA 1mg IV/IO every 3-5 min , unstable bradycardia 0.5mg IV/IO every 3-5 min MAX 3mg Pediatric: child 0.02 mg/kg IV/IO (2X for 2nd dose) min single dose 0.1mg max single dose 0.5mg (max dose child 1mg) ,
