  • Facotr Xa + Ca++, and phospholipids + factor-V → forms prothrombin-activator-complex → converts pro-enzyme prothrombin (inactive) → thrombin (active enzyme) → thrombin clips fibrinogen → causes release of fibrin monomers → fibrin monomers polymerize → stabilize initial platelet plug

  • These slow the clotting time + suppress coagulation
  • Examples of drugs in this class:

-Heparin, LMW heparin, Fondaparinux

-Warfarin (for slow + prolonged + oral anti-coagulation)

-Direct thrombin inibitors (Lepirudin, bivalirudin, argatroban)

  • Thought these would have fever SE than warfarin
  • Have fewer drug-drug interactions than warfarin 
  • There is NO accurate testing for their affect on coagulation (cannot use PT or aPTT)
  • These are oral medications
  • BUT these have been reported to cause SEVERE bleeding and their use is being reconsidered b/c they are unable to be monitored

How does platelet activation work?
Definition
  • Platelet activation is iniated by ADP → ADP binds and leads to conformational change in platelet glycoprotein-IIb/IIIa → this activates the platelets and allows for binding to fibrin
