Question:
A 55-year-old male is brought to the emergency room with rigid, painful, red swelling in the anterior part of his neck. He smells of alcohol and is incoherent. He has a fever of 102 degrees. His history is unreliable due to intoxication. Examination shows a tense, firm, red suppurative mass in the submandibular area. Pharyngeal examination reveals poor dentition and malodorous breath. Sinuses are nontender to palpation. Lungs are clear to auscultation bilaterally. Soft tissue neck films reveal significant soft tissue swelling and moderate airway compression. Ludwig's angina is a dangerous cellulitis of the submandibular area that usually originates from poor dentition, particularly the rearmost molars in alcoholics. The infection causes swelling of the pharyngeal structures and can lead to airway compromise. Tracheostomy is sometimes needed to maintain an airway. The organisms involved are typically Staph or Strep species, but mixed anaerobic infections also occur. Intravenous antibiotics and airway support are the main treatments. |
