pathophysiology:
· a.k.a. B12 deficiency (can only be absorbed in the gut with the aid of intrinsic factor, vital for proper RBC production by bone marrow)
o IF is secreted by parietal cells in the gastric mucosa and binds to B12
· characterized by macrocytic, nucleated immature erythrocytes (“production” problem)

Etiology:
· dietary insufficiency is rare
· malabsorption of B12 (most common)
o autoimmune reaction (destroys parietal cells in mucosa)
o chronic gastritis (alcholics- parietal cells destroyed)
o surgery (gastrectomy due to cancer, bariatric surgery, etc.)
· B12 deficiency malabsorption results in macrocytic RBCs- bone marrow “factory” makes “large, sloppy” RBCs, varied sizes, increased variation in size of RBCs (DRW on CBC): anisocytosis

S/S (in addition to normal anemia signs)
· Macroglossia (enlarged tongue)
· Achlorhydria: dec. HCl production from mucosa (confirms pernicious anemia diagnosis)
· Neurologic effects
o Tingling or burning sensation caused by peripheral demyelination
o Loss of coordination (ataxia)

Diagnostic tests:
· Microscopy reveals large megaloblastic cells
· Dec. B12 level
· Gastric hypochlorydria or achlorhydra (HCl) àconfirms gastric atrophy of parietal cells

Trmt:
· Oral supplements: prophylaxis ONLY
· B12 injections for diagnosed pernicious anemia

· Bone marrow biopsy: low reticulocytes & stem cells

· If you have the trait, you are slightly resistant to malaria
During hypoxia, dehydration, stress: Cells “sickle”
àcause hypoxia and vascular occlusions; very painful
àdamaged RBCs cause increased hemolysis

S/S
· Severe anemia causing pallor, weakness, tachycardia, and dyspnea
· Hyperbilirubinemia: jaundice skin and icteric sclera
· Splenomegaly (spleen filters blood; becomes congested)
· Vascular occlusions and infarctions lead to periodic painful crisis (“sickle cell crisis”)
· Growth and development delays
· Congestive heart failure: from anemia, increased viscosity, & vascular congestion of occlusions

· Hemoglobin electrophoresis
· DNA testing: for homozygous HgS (heterozygous- no anemia)

Trmt:
· Hydrea to reduce sickling crisis
· Avoidance of strenuous activities & high altitudes
· Prevent dehydration, acidosis, infection, exposure to cold
· Prophylactic antibiotics
· In severe cases life expectancy may be as young as 20 years old

Pathophysiology:
· Involves both excessive bleeding due to a deficiency of clotting factors and excessive clotting (thrombus)
· With cellular damage, both fibrinogen (clot former) and thromboplastin (fibrinolysis) are activated
· Many small emboli/thrombosis cause tissue damage, while clotting factors (proteins) are used up
· **Pt simultaneously experiences embolic/thrombotic tissue ischemia (inappropriate clotting) AND bleeding from inability to make clot (exhausted clotting proteins) POOR PROGNOSIS

Pathophysiology:
· Undifferentiated, immature, nonfunctional leukocytes present and multiplying uncontrollably in the bone marrow. Large amounts are released into the general circulation. Very high leukocyte levels (WBC) on CBC.
· These cells then infiltrate the lymph nodes, spleen, liver, brain, and other organs, causing lymphadenopathy (swelling of lymph nodes).

· Multiple nodes involved, with NO orderly spread
· Often affects both sides of body at time of diagnosis
· Intestinal nodes and abdominal organs involved
· Non-localized tumors more difficult to treat
· DOES NOT contain Reed-Sternberg Cells on biopsy
· Prognosis is improving, but still poor
