All 3 Na channel blockers plus procainamide is k channel blocker. Lidocaine weakest, flecanide strongest.
Highest affinity when Na Channel open prevents re-entry tachycardia and delays DADS.
Increases QRS interval, may causes mono-Vtac via increase in calcium lvls (QRS=ventricular conduction by NA, QT is ventricular repolarization by K)
procainamide may also causes torsalsdepoints (early after depolar) via k.
Lidocaine: small negitive ionotrope, supress vtac Flecainide: huge negitive ionotrope drug of choice low risk a fib (pocket) procainimide:use vtac may cause lupup-like |
