Cards

Term
precocious sexual matruation
Definition
congenital disorders
extra sex hormone production
very rare- where puberty occurs before 6 in AA and 7 in White, and 9 years in boys of either race
could be idiopathic cause or central nervous system tumor
Term
delayed puberty
Definition
see this more often in boys
95% of time is no cause of concern, just slow
5% systematic disease or other problem
Term
dysmenorrhea
Definition
painful menstruation
Term
polycystic ovary syndrome
Definition
2 of these

Polycystic ovary
Oligo-anovulation-no ovulation
Hyperandrogenism- male sex hormones

hyperinsulinemia has key role
associated with hypertension, lipiemia, hyperinsulemia, dyslipidemia (high cholesterol)
Term
pelvic relaxation disorders
Definition
Cytoscele decent of the bladder into the vaginal canal
Can bulge outside the vagina
Main manifestation of this is stress incontinence-leakage of urine in laughing coughing

Rectocele: descent of bladder into rectal canal
Difficult to have a bowel movement
Term
prolapsed uterus
Definition
descent of cervix or entire uterus into the vagina

3 different degrees
1. descent of uterus or cervix into vagina
2. protrudes through itroitus
3. vagina is completely everted due to prolapsed uterus
Usually no signs and symtptoms
And 3 may feel fullness, protrusion through vagina
Term
leiomyomas (uterine fibroids)
Definition
Benign tumors –most common
Related to estrogen levels
Shrink after menopause
Increase during pregnancy, decrease after childbirth
Not in many caucasians
Usually occur in multiples in fundus of uterus
Classified by location in uterine wall
Term
endometriosis
Definition
Common disorder of the female reproductive system
Functioning endometrial tissue that implants outside the uterus
Implantation responds to fluctuations in the menstrual cycle
Proliferation, breakdown, bleeding of tissues
Can lead scarring, lesions, fibrosis, can lead to infertility
Sites: Ovaries, pelvic peritoneum rectovaginal septum uterine ligaments
s/s: infertility (1/3), pain, dysmenorrhea, dyskiseia (pain on urination), constipation, abnormal vaginal bleeding
Term
priapism
Definition
Prolonged penile erection
Urologic emergency
Associated with erectile dysfunction medications

Not associated with sexual arousal
Very painful

60% of cases are ideopathic
40% spinal cord trauma, sickle cell disease, leukemia, pelvic tumors, and impotence related medications
If not treated quickly can lead to permanent impotence.
Term
varicocele
Definition
Dilation of a vein within the spermatic cord
“bag of worms”
Decreases blood flow through the testis-Infertility

Common finding of infertile men
Not diagnosed until trying to conceive
Can be painful or tender depending on location

Cuased by imcompentant or congentially absent valves in spermatic veins
Causes blood to pool
Decreased flow through testis

If varicoseal is not severe, fertility issues may not happen
Term
hydrocele
Definition
Collection of fluid in the tunicovaginalis

Scrotal swelling most common cause
About 6% of male newborns

Congenital issue, resolves spontaneously in first year
Imbalances in secreting and absorbing of scrotal tissues
Amount of edema can lead to atrophy
Term
cryptorchidism
Definition
Failure of the testes to descend into the scrotum
Common congenital condition
If untreated may lead to infertility

Peds check testes every visit to make sure they’re approriately distended
Premature infants 20-30 % of infants
3-5% of full term infants
Main remain in abdomen, inguinal canal or, puboscrotal junctions

Untreated can lower sperm count due to higher temperatures
Leads to decrease in fertility
Doesn’t prevent puberty
30-50x greater chance of testicular cancer if this is left untreated
Term
torsion of testis
Definition
Testes rotate and twist, cause arteries and veins to twist, decreasing blood supply to testis, causes vascular engorgement and ischemia and leads to scrotal swelling and pain
Acute scrotum
Most common in neonates and adolescents at puberty
Surgical emergency
Onset is spontaneous or occurs after physical exertion or trauma
Term
prostatitis
Definition
Inflammation of the prostate
Characterized as:
Acute bacterial
Chronic bacterial
Nonbacterial

Acute bacterial
ascending infection of urinary tract
inflammatory response
large tender, firm, boggy prostate
systematic signs of infection like fever and chills
diagnosis is urine culture
Chronic bacterial
recurrent urinary tract signs and symptoms of bacterial symptoms in urine
s/s similar to 1
prostate only slightly enlarged
fibrotic due to recurrent infections
Nonbacterial
most common prostatis
inflammation without evidence of bacterial infection
reflux of sterile urine into ejaculatory ducts
ejaculatory pain, urinary signs and symptoms
prostate is normal
Term
human papillomavirus
Definition
choldyloma acuminota
most common viral STD in USA
cervical cancer and anorectal and squamous cell in males
causes genital warts
pap smear for diagnosis
vaccines for HPV
Term
trichomonas
Definition
parasite
lower genital tract infection
common in urethra of males
vagina of females
asymptomatic until pain on intercourse, dysuria, spotting
Term
herpes
Definition
vaginal secretions that look similar to sperm
HV1: cold sores

HSV2: genital ulcerations
Epidemic status
Neontal infection can occur at all stages (inuerum, intrapartum post partum)

Vescicles that lead to ulcerations
Usually a latent phase until it is reactivated
Outbreak can come and go
Usually some triggers that stimulate one to reactivate and produce the ulcerations
stress, sun exposure, hormonal fluctuations and illness
tissue culture and antibody testing of serum to diagnose
